FEES

  $125 per 50-minute session

All Major Credit/Debit cards including HSA/FSA.  Payment will be deducted at the time that services are rendered. 

Evolve Family Counseling may be out-of-network for your insurance provider. A superbill, which is an itemized form used by healthcare providers will be provided for you to submit to your insurance provider. Reimbursement may be possible at the discretion of your insurance provider based on your current policy.
